Winter Break Food Assistance Application

Harlow Serves would like to provide your child with food assistance over Winter Break. This program is for students currently enrolled at Holt Elementary.

To receive grocery items for ten weekdays (breakfast and lunch) of Winter Break, please fill out the form below. If you have more than one child at Holt elementary, please fill out the form SEPARATELY FOR EACH CHILD. Please apply by December 7th.

Food bags will be available for pickup from Harlow Serves (at North Park Community Church, 3484 Harlow Road) at the following times:
7:30am-8:30am and 2:00pm-3:00pm on Wednesday, December 18th and Thursday December 19th. If you need to arrange a pickup outside of these times, please email contact@harlowserves.com
